You play as the same character from MANIC Episode 1.
After the ending of Episode 1, you awaken in a distorted, unknown world, a nightmare realm shaped from your memories, violence, and something darker.
This place isn’t real. It’s the Unknown World — a merging of your mind and Quake’s chaos.

Your skin is gray like the dead, your blood burns like madness, and your only goal remains the same:

Find Him. Kill Everything. Get Your Revenge Back.

Trapped in the Quake World and cursed with unstable powers, you must fight your way through twisted halls, broken memories, and the echoes of your past. The boundaries between life and death blur as monsters, traps, and hidden secrets test your every step.
